MIDDLE SCHOOL YOUTH FORMS

 

All Middle School Youth must complete the attached forms below in order to attend any of the youth events at Mt. Pleasant Presbyterian Church. 

You can download these forms, print them complete them and drop/mail them back to the church.  Please put attention Stacy Hanling.

 

GENERAL FORMS

  • Youth Medical Release Form    (2 sided form)
  • Adult Medical Release Form      (for Chaperones)
  • Parent Covenant Form  (Read and signed by parent)
