For Church Ministries:  Food will be prepared and your ministry will be billed for the number of people you indicate on this form.  If that number should change, please contact Tom Dean immediately.  Any money collected for the meal is to reimburse your ministry.  All collected funds for food service should be submitted to the Finance Office.
